This Avon Primary Care Research Collaborative (APCRC) seminar will include brief presentations from four Bristol Health Partners Health Integration Teams (HITs) working on new services aimed at keeping people out of hospital. The HITs presenting include:

These will be followed by a Q&A panel chaired by Peter Brindle, Medical Director (Clinical Effectiveness), Bristol, North Somerset and South Gloucestershire Clinical Commissioning Groups (CCGs).

The Bristol Health Partners’ Health Integration Teams bring together health and care professionals, managers, patients and members of the public, commissioners, local government, third sector organisations and researchers to address pressing health challenges and improve health outcomes. HITs aim to bridge the gap between research and practice.

There will be networking and light refreshments from 12.15pm, with the seminar starting at 1pm.
