End of Life Matters HIT

An all-age initiative dedicated to improving end of life care through close collaboration with local communities across the South West.

End of life affects us all — at every age, and in every community. The End of Life Matters HIT has been created to improve care and support for people approaching the end of life and for the families and communities around them across the South West.

Our goal is to make care more joined-up, fair and responsive to what matters most. To achieve this, we will:

  • Work alongside communities to understand their experiences, priorities and ideas.
  • Bring people and organisations together – from the NHS, hospices, social care, universities and charities – to design solutions collaboratively.
  • Build on research and share learning so that evidence and experience shape lasting improvements in care and support.

By combining the voices of communities with professional expertise, the End of Life Matters HIT will create the conditions for new ideas to grow, drive meaningful change, and strengthen support for everyone at the end of life. Everyone is welcome to contribute, because every voice matters.
