End of Life Matters HIT

An all-age initiative working to improve care for people approaching end of life, and for their communities across the South West.

The End-of-life Matters Health Integration Team (ELM HIT) aims to bring people and organisations together to design care that is more joined-up, fair and shaped by what matters most to those who need it.

The HIT will connect local communities with the NHS, hospices, social care, universities, and charities. By listening carefully to people’s experiences, and using these to guide decisions, the HIT will help make care more equitable and consistent. We will also use new ways of collecting and studying experience data so we can spot patterns, share learning, and improve support for everyone.

By combining personal stories with professional knowledge, the HIT aims to be a catalyst for evidence-informed care to create lasting change and better support for all communities.
