The survey is aimed at anyone in England who has received a diagnosis of dementia in the last two years, between November 2014 and November 2016. It’s also for anyone in England who currently provides unpaid care or support to a family member, friend or neighbour who has been diagnosed with dementia in the same period. The survey has 38 questions, so please respond to the survey if you’re eligible.
